Order/Quote Form

If you receive a CGI Error when you submit this form it is because your Internet Security software is blocking the form submission. To permit the submission in Norton's Internet Security program follow the steps below:

1) Open Norton Internet Security
2) Click on Privacy Control
3) Click on Configure
4) Click on Advanced
5) Click on Add Site (type in www.lewispaper.com)
6) Uncheck all boxes in Global Settings
7) Click on Permit for all options available
8) Click OK, Click OK again
9) Close program

You will only need to do this one time.

Order: Will Call UPS AM Delivery (where available) PM Delivery (where available)

Quote: Phone Fax     Be sure to provide us with either your Phone or Fax number.
You must also supply a city and state if you want us to include shipping charges with the quote.

5. Payment Method: On Account Credit Card Delivery COD Check

We accept Visa, Master Card and American Express.

You must call in your Credit Card No. the first time you want to place an order using that card.
